  3. Select which categories you would like to enter.
  4. Complete entry form and fax it back to: 702-253-9985. Deadline: May 18, 2011
  5. Ready-to-Drink entrant must ship at least 6 bottles of each entry to Wilbur Curtis Corporate Headquarters – the judging location. Case packs are preferred as they are easily identifiable.
  6. Foodservice entrant must submit tea in its original, sealed foodservice case with labeling indicating its contents (ie. (32) 3oz. pack) to Wilbur Curtis Corporate Headquarters-the judging location.
  7. Send samples to: (Must be received by May 20, 2011)
